2. Nurse Practitioner/ Physician Assistant Credentialed

Four Rivers Community Health Center is hiring an Nurse Practitioner/ Physician Assistant for the Rolla, MO office location.

POSITION SUMMARY: An exempt position responsible for providing primary care services, including assessing, diagnosing, prescribing, treating, and educating patients. Works closely with physicians and other clinicians in a team approach to patient care. May specialize in a specific type of clinical care, e.g., pediatrics, geriatrics.

Supervision Exercised:

May share in the supervision of RNs, LPNs, and/or medical assistants, depending on departmental organization

Supervision Received:

Reports to the Medical Director.


Assesses patient status by obtaining health history through patient/family interviews and chart reviews; assessing presenting illness, risk factors, and family history, psychosocial situation, and cultural factors; and performing appropriate physical examination.

Orders/performs appropriate laboratory diagnostic and other screening tests. Seeks other information as needed, including consultation with physicians and other clinicians, for evaluation of illness. Integrates data to determine diagnosis and therapeutic plan, including identification of any health risks.

Develops and implements treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws. Handles minor lacerations. Instructs patient/family regarding medications and treatments. Educates patients regarding health promotion/illness prevention. Recommends appropriate community resources to meet patient/family needs. Communicates appropriate case management information to other professionals and community agencies. Prepares documentation for medical records including updating patient medical chart by posting examination and test results, diagnosis, medications, and treatment in written/computerized manner.

Participates in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures. Acts as consultant/collaborator to clinical team on area of specialty. Complies with patient confidentiality requirements. Promotes patient advocacy.

Collaborates with physicians in managing acute and long-term medical needs of patient. Provides monitoring and continuity of care between visits according to treatment plan including triaging patient calls/e-mails.


BSN required, MSN preferred. Successful completion of advanced practitioner training and certification as a practitioner (general or specialized) from an approved nurse practitioner program.

Minimum three years of professional nursing experience in a clinic setting, plus two years of nurse practitioner experience.

Current state registered nurse license. Current state license as an advanced registered nurse practitioner with prescriptive authority.

Current CPR certification.

Provides primary comprehensive dental care services in the prevention, diagnoses and treatment of dental diseases, injuries and/or deformities; performs related work as required.                       

Essential Duties and JOB RESPONSIBILITIES:

·Examines, diagnoses, treats and prescribes medications to patients of the Center.

·Provides dental services including, but limited to the following:

  • Preventive dental services, i.e. prophylaxis, scaling and root planning, fluoride treatment, varnishes and/or sealants, and radiologic procedures
  • Operative dentistry, i.e. amalgams, composites, and crowns
  • Pedodontics services
  • Oral surgery, i.e. simple to moderately complex extractions
  • Endodontics on selected cases determined by professional judgment
  • Emergency dental care.

·Maintains dental records for each patient in accordance with the Center’s policies and procedures.

·Provides inpatient dental services within scope of training and ability.

Job Summary: As the QRMD, daily duties include being responsible for ensuring standards of safe, effective, and professional care are maintained at all times, assisting in the development of policies and procedures, and participating in service/program evaluations and the claims management process and claims point of contact.

Risk Management

· The QRMD oversees the claims management process and processing of claims related activities and serves as the claims point of contact in the claims management process

· The Claims Management process is the daily responsibility of the QRMD and are responsible for following the FTCA claims management process located in the FTCA manual

· Responsible for all Quality and Risk Management reporting for PCMH, UDS, HRSA site visit requirements, 340B and any other organizations

Performs, manages, and tracks risk assessment in all day-to-day activities within the clinic.

· Promote and maintain a safe work environment for all staff and ensure that all Risk Management and Health & Safety strategies, policies and procedures are implemented

· Works in daily collaboration with clinic managers and clinic directors to assist the Four Rivers team with quality and risk management activities in accordance with Four Rivers mission, vision, and values

· Creates and modifies policies to comply with safety legislation, HIPAA, and industry practices. Ensuring compliance with state and federal laws, licensing, organizational recognition standards, MPCA and network insurer quality programs requirements

General Summary:

Job summary:

Assists providers with patient care and follow-up providing direct and indirect patient care in a clinical setting. Duties to include, but not limited to, interviewing patients, obtaining vital signs, preparing patients and assisting providers with physical exams and/or treatment. Continuing responsibilities including charting and clerical work as necessary to oversee phone prescriptions and refills to pharmacies. Additionally, the LPN or CMA will perform triage with patient phone calls to identify emergent and non-emergent situations per triage policy.

Essential Job Responsibilities:

  • Demonstrates ability to carry out skills as outlined in nursing procedures for clinical areas including administration of prescribed medications and treatment in accordance with nursing standards and clinical policy.
  • Completes orientation checklist. Completes specific competency skills checklist
  • Obtains, verifies, and records health history of patients. Gathers pertinent health data through physical assessments and biological samples. Collects specimens for laboratory analysis.
  • Provides basic patient care as ordered. Administers medications and treatments as directed. Assists other health care professionals in performing exams, diagnostic procedures and treatments, and minor medical procedures. Adheres to infection control/safety guidelines and confidentiality policies.
  • Documents assessments, interventions, patient/family responses, and medication dispensed/prescribed, and test results in medical records. Complete appropriate treatment questionnaires. Complete billing charges at end of each treatment. Ensure that ALL activities completed are charted within the patient’s medical record.
  • Monitor incoming phone calls: All incoming calls are to be answered and addressed. If unable to receive call due to direct patient care requirements, voicemail messages will be returned as per policy and procedure directs return calls.
  • Promotes wellness by providing patient education materials, communicating provider orders/instructions.
  • Ensure clinic areas are kept stocked at acceptable levels. Disinfect and clean exam/treatment rooms between patients. Infectious waste/sharps containers are emptied when full. Dirty linen will be removed from treatment areas daily.

Education: High school diploma or equivalent. Graduation from an approved practical nursing program.

Experience: Minimum one year of LPN or MA experience; clinic experience preferred.

7. Dental Assistant Credentialed

The Dental Assistant performs duties such as: prepare the patient for treatment, assist dentist in performing their treatment to the patient in preventative and restorative dentistry or oral surgery, prepare materials and equipment for treatment and have them ready for the dentist’s use, take x-rays and assist the dentist in laboratory work. May perform other related work included, but not limited to, making appointments, ordering supplies and aiding with the dental billing. The position is at the Four Rivers Clinic in Rolla, MO. This position is fulltime/ 40hrs a week.

8. Billing Specialist Noncredentialed

Essential Job Responsibilities:

  • Reviews patient insurance, billing, collections, and data processing to ensure accurate patient billing and efficient account collections.
  • Review patient accounts/claims to ensure billing claims and paperwork for claims are in accordance with guidelines.
  • Operate software system for billing, posting remits, and claims research
  • Coordinate and collect necessary information from staff or payers for claim adjudication for both medical and dental claims.
  • Verify accuracy of billing data and revise errors; resubmit revised claims.
  • Recognize problem accounts and notify appropriate staff or supervisor to assist in problem resolution.
  • Monitors commercial, Medicaid, Medicaid Manage Care, Medicare insurances aging reports and takes such steps as necessary to guarantee payment of past due claims.
  • Weekly review and verify claims are processing through clearinghouse correctly.
  • Weekly verify/review claims are hitting and are being paid correctly in payer portals.
  • Interact with staff, clients, payer and Agencies to answer questions, obtain information, and resolve issues.
