Four Rivers Community Health Center is hiring an Nurse Practitioner/ Physician Assistant for the Rolla, MO office location.

POSITION SUMMARY: An exempt position responsible for providing primary care services, including assessing, diagnosing, prescribing, treating, and educating patients. Works closely with physicians and other clinicians in a team approach to patient care. May specialize in a specific type of clinical care, e.g., pediatrics, geriatrics.


Supervision Exercised:

May share in the supervision of RNs, LPNs, and/or medical assistants, depending on departmental organization

Supervision Received:

Reports to the Medical Director.

ESSENTIAL JOB RESPONSIBILITIES:

Assesses patient status by obtaining health history through patient/family interviews and chart reviews; assessing presenting illness, risk factors, and family history, psychosocial situation, and cultural factors; and performing appropriate physical examination.

Orders/performs appropriate laboratory diagnostic and other screening tests. Seeks other information as needed, including consultation with physicians and other clinicians, for evaluation of illness. Integrates data to determine diagnosis and therapeutic plan, including identification of any health risks.

Develops and implements treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws. Handles minor lacerations. Instructs patient/family regarding medications and treatments. Educates patients regarding health promotion/illness prevention. Recommends appropriate community resources to meet patient/family needs. Communicates appropriate case management information to other professionals and community agencies. Prepares documentation for medical records including updating patient medical chart by posting examination and test results, diagnosis, medications, and treatment in written/computerized manner.

Participates in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures. Acts as consultant/collaborator to clinical team on area of specialty. Complies with patient confidentiality requirements. Promotes patient advocacy.

Collaborates with physicians in managing acute and long-term medical needs of patient. Provides monitoring and continuity of care between visits according to treatment plan including triaging patient calls/e-mails.

MINIMUM QUALIFICATIONS:

BSN required, MSN preferred. Successful completion of advanced practitioner training and certification as a practitioner (general or specialized) from an approved nurse practitioner program.

Minimum three years of professional nursing experience in a clinic setting, plus two years of nurse practitioner experience.

Current state registered nurse license. Current state license as an advanced registered nurse practitioner with prescriptive authority.

Current CPR certification.

2. OB/GYN Credentialed

Four Rivers Community Health Center is hiring an OB/GYN for the Rolla, MO office location.

To support the Mission, Vision, and Values of Four Rivers Community Health Center an OBGYN with us will provide high-quality primary care services utilizing evidence-based practices and adhering to quality guidelines set forth by HRSA and other health center program requirements such as PCMH and PCHH.

Duties

    • Provides Quality and comprehensive prenatal and gynecologic care to patients
    • Responsible for providing consultations requested by primary care providers
    • Will participate in an on call schedule starting at 1 in 4 with potential of moving to 1 in 5
    • Participates in continuing medical education as required for licensure and skills maintenance
    • Works closely with primary care providers to ensure that the patient is getting the best in comprehensive medical care

Qualifications

  • Diploma from an accredited medical school
  • Licensed to practice in Missouri or willing to obtain Missouri license
  • Board certified or board eligible status

3. Dentist Credentialed

Provides primary comprehensive dental care services in the prevention, diagnoses and treatment of dental diseases, injuries and/or deformities; performs related work as required.                       

Essential Duties and JOB RESPONSIBILITIES:

·Examines, diagnoses, treats and prescribes medications to patients of the Center.

·Provides dental services including, but limited to the following:

  • Preventive dental services, i.e. prophylaxis, scaling and root planning, fluoride treatment, varnishes and/or sealants, and radiologic procedures
  • Operative dentistry, i.e. amalgams, composites, and crowns
  • Pedodontics services
  • Oral surgery, i.e. simple to moderately complex extractions
  • Endodontics on selected cases determined by professional judgment
  • Emergency dental care.

·Maintains dental records for each patient in accordance with the Center’s policies and procedures.

·Provides inpatient dental services within scope of training and ability.

·Reviews radiological and medical reports and incorporates findings into plan of care.

·Serves in rotation for after-hours call system.

MINIMUM QUALIFICATIONS:

·Graduate of an accredited school of dentistry and holds current license as a dentist in the State of Missouri, as well as current DEA, and BNDD

·Demonstrated professional commitment to providing services to medically under-served persons

·Ability to communicate effectively both orally and in writing

·Computer literate in electronic mail, word processing, and office management system software

·Adequate vision, speech and hearing to meet core duties of the position

·Ability to reach, stand, stoop, walk, and lift 25 pounds

4. LPN/CMA Credentialed

Due to growth of our clinic we are looking for a dedicated and enthusiastic LPN or MA to join Four Rivers Community Health Center's team at our Rolla location.

General Summary:

Job summary:

Assists providers with patient care and follow-up providing direct and indirect patient care in a clinical setting. Duties to include, but not limited to, interviewing patients, obtaining vital signs, preparing patients and assisting providers with physical exams and/or treatment. Continuing responsibilities including charting and clerical work as necessary to oversee phone prescriptions and refills to pharmacies. Additionally, the LPN or CMA will perform triage with patient phone calls to identify emergent and non-emergent situations per triage policy.

Essential Job Responsibilities:

  • Demonstrates ability to carry out skills as outlined in nursing procedures for clinical areas including administration of prescribed medications and treatment in accordance with nursing standards and clinical policy.
  • Completes orientation checklist. Completes specific competency skills checklist
  • Obtains, verifies, and records health history of patients. Gathers pertinent health data through physical assessments and biological samples. Collects specimens for laboratory analysis.
  • Provides basic patient care as ordered. Administers medications and treatments as directed. Assists other health care professionals in performing exams, diagnostic procedures and treatments, and minor medical procedures. Adheres to infection control/safety guidelines and confidentiality policies.
  • Documents assessments, interventions, patient/family responses, and medication dispensed/prescribed, and test results in medical records. Complete appropriate treatment questionnaires. Complete billing charges at end of each treatment. Ensure that ALL activities completed are charted within the patient’s medical record.
  • Monitor incoming phone calls: All incoming calls are to be answered and addressed. If unable to receive call due to direct patient care requirements, voicemail messages will be returned as per policy and procedure directs return calls.
  • Promotes wellness by providing patient education materials, communicating provider orders/instructions.
  • Ensure clinic areas are kept stocked at acceptable levels. Disinfect and clean exam/treatment rooms between patients. Infectious waste/sharps containers are emptied when full. Dirty linen will be removed from treatment areas daily.
  • Any other duties as assigned.

Education: High school diploma or equivalent. Graduation from an approved practical nursing program.

Experience: Minimum one year of LPN or MA experience; clinic experience preferred.

5. Dental Assistant Credentialed

Assist dentist with patient care and follow-up providing direct and indirect patient care. The Dental Assistant performs duties such as: prepare the patient for treatment, assist dentist in performing their treatment to the patient in preventative and restorative dentistry or oral surgery, prepare materials and equipment for treatment and have them ready for the dentist’s use, take x-rays and assist the dentist in laboratory work. May perform other related work included, but not limited to, making appointments, ordering supplies and aiding with the dental billing. The position is at the Four Rivers Clinic in Rolla, MO. This position is fulltime/ 40hrs a week.
